Enrollment Information
Program Eligibility
To be eligible to participate in a Summer Abroad program, you must:
- be 18 years or older
- have completed 15 accredited college units
- be a University of California student OR have permission of the instructor to enroll
- be in good academic standing with a GPA of 2.0 or higher
- be in good disciplinary standing with your academic institution
Permission to Enroll for Non-University of California Students
UC Davis Summer Abroad welcomes students from outside the University of California
system (UC - Berkeley, Davis, Irvine, Los Angeles, Merced, Riverside, San Diego,
San Francisco, Santa Barbara, and Santa Cruz). If you are not currently matriculated
at a UC, you will need permission to participate in the program. To request permission,
please email Summer Abroad (summer-abroad@ucdavis.edu)
the information below.
- the name of your college/university
- your class level (i.e. Junior)
- your current GPA
- your major and minor
- a list of courses you have taken that are related to the program topic/courses
- the reasons you want to participate in the program
How to Enroll
Summer Abroad programs fill on a first-completed, first-reserved basis, so submit
your materials early to secure your spot in one of our 2010 programs. The enrollment
process is the same for all UC students. If you are a student from outside the University
of California, you will need to request permission to enroll before completing your
enrollment packet.
Create your online profile and complete the online enrollment forms beginning October 1, 2009.

Complete your online Profile
After completing the online Student Profile and signing the online Release of Information,
you will have the option of selecting programs that interest you. You will not be
obligated to enroll in these programs but will receive program updates via email,
including announcements about information sessions with the instructor and news
on fees and accommodations. You can check as many programs as you like. To enroll
in a program, you must complete all the required enrollment forms and materials
(Step 3 below).

Read the 2010 Summer Abroad Student Handbook (SASH)
The SASH contains important information on passports, visas, health, safety, vaccines,
communication, and enrollment procedures. All applicants need to read it!

To secure your enrollment, complete and submit all items on your Enrollment Checklist beginning January 4, 2010. All materials will be available by December 15, 2010.
- Create a Summer Abroad Student Profile
- Sign (electronically) the Release of Information Form
- Complete the Contact Information (online)
- UC Davis students sign (electronically) the Student Judicial Affairs Release
Form
- Non-UC Davis students print the form and take it to the official at their campus.
- Read the Summer
Abroad Student Handbook (SASH)
- Sign (electronically) the Audio/Photo/Video Release Form
- Complete (electronically) the Allergies & Dietary Request Form
- Financial Aid students complete (electronically) the Request for Deferment
- Select your courses (electronically)
- Complete (electronically) the Accommodations Preference Form
- Submit your roommate request
- Complete (electronically) the Participant Background Form for your instructor
- Upload (electronically) a copy or picture of your passport, or proof that
you have applied for a passport
- Review the Program Reminders and "sign" online
- Download and complete the following forms to the Summer Abroad
office:
- All Contracts (Participant Contract, Waiver of Liability, Authorization for Treatment
& Release of Health Information)
- Enrollment Cover Sheet & Deposit Payment Form
- Health Clearance Forms (Refer to Summer Abroad Health Clearance Instructions)
- Submit items above with the following materials
- $300 non-refundable deposit (check or money order made out to "UC Regents")
- Unofficial copy of transcript (print from online)
- 1 passport-style photo (2” x 2” color headshot) with your name, program, and 2009
written on the back
- Send packet by mail or bring to:

Health Clearance Instructions
All students enrolling in a Summer Abroad program must complete the Health Clearance
process. You will need to submit the forms indicated below to Summer Abroad at the
Education Abroad Center with your enrollment packet.
Students are encouraged to seek a health-clearance as soon as they have determined
their destination. Follow the instructions below to get this important part of your
enrollment packet underway today!
UC DAVIS STUDENTS
Option 1: Online via Cowell Student Health Center
All UC Davis students have the option of using the Cowell Student Health Center.
As of September 30, 2009, the health clearance review costs $20. Students traveling
to a destination that requires a travel clinic will pay an additional $15.

If you are applying to a program in Argentina, Brazil, China, Ghana, India,
Trinidad & Tobago, or South Africa, follow the instructions below. If you
will be traveling to another country, go to Section B.
The Travel Clinic nurse will review with you the recommended precautions for travel
within your program’s country. UC Davis students should follow these steps if travel
clinic is required –
- Go to Cowell Student Health Center IN PERSON to make a Travel Clinic appointment
and pick-up a Travel Clinic Questionnaire.*
- Fill-out the entire Travel Clinic Questionnaire and take it to your Travel Clinic
appointment.
- After your Travel Clinic appointment, login to the Health e Messaging online system
through the Cowell Student Health Center website
http://healthcenter.ucdavis.edu/hem/index.html using your Kerberos ID and
password.
- Enter your UC Davis student ID number.
- Click the "Messages page" link on the Health e Messaging homepage.
- Click the "New Message" button near the top of the Messages page.
- Select the "Request clearance for Travel Abroad" option and click “Continue.”
- Select the "Summer or Quarter Abroad" option and click "Continue."
- Follow the instructions to complete the remainder of the Health History Questionnaire.
- Within two weeks of submitting your Health History Questionnaire you will receive
an email via Health e Messaging confirming that your information has been reviewed
and you have been cleared for travel abroad.
- Print out all pages of the confirmation email (that should include the information
you submitted in the Health History Questionnaire).
- Submit the Health e Messaging email with your enrollment packet to the Summer Abroad
office.
NOTE: If you waived your SHIP insurance by providing proof of private health
insurance, you will be charged the full cost of any recommended vaccinations or
travel medications – and should therefore seek these services from your primary
care physician.

If you are going to a country not listed above, follow the instructions
below.
- Log-in to the Health e Messaging online system through the Cowell Student Health
Center website at http://healthcenter.ucdavis.edu/hem/index.html
using your Kerberos ID and password. NOTE: Please be aware that the online questionnaire
will request your immunization history. You will not be able to save the questionnaire
and complete it at a later date.
- Enter your UC Davis student ID number
- Click the "Messages page" link on the Health e Messaging homepage
- Click the "New Message" button near the top of the Messages page
- Select the "Request clearance for Travel Abroad" option and click "Continue"
- Select the "Summer or Quarter Abroad" option and click "Continue"
- Follow the instructions to complete the remainder of the Health History Questionnaire.
If you are applying for Argentina, Brazil, China, Ghana, India, Trinidad & Tobago,
or South Africa programs, go to Section A for instructions.
- Within two weeks of submitting your Health History Questionnaire you will receive
an email via Health e Messaging confirming that your information has been reviewed
and you have been cleared for travel abroad. If you do not receive an email within
two weeks, please contact Cowell Student Health Center.
- Print out all pages of the confirmation
email (that should include the information you submitted in the Health History Questionnaire)
- Submit the Health e Messaging email with your enrollment packet to the
Education Abroad Center.
Option 2: Paper copy via Private Physician
If you are a UC Davis student with private health insurance (i.e. you submitted
a SHIP waiver and receive health insurance from a private health care provider),
you may choose to see your physician to complete the Health Clearance Forms. To
do this, see instructions for Non-UC Davis students.
NON-UC DAVIS STUDENTS
Paper Copy via Private Physician or non-Davis Campus Health Center
You may see your private physician or your campus health clinic to complete the
Health Clearance process. In order to do this, please
download and print the Summer Abroad Health Clearance Form. Complete this
form and take it with you to your appointment. Your doctor will review the information
with you, talk to you about travel health issues, provide you with the required
immunizations for your destination and clear you for travel abroad by signing the
last page of the Health Clearance Form. It is your responsibility to return this
form with your Enrollment Packet to:
